Complimentary Teeth Whitening Consultations

Conditions of offer:
(1) Patient must be over 16 years of age
(2) Offer does not include exam, check-ups, cleaning, or the actual teeth whitening procedure.

Receive 50% off your cleaning and fluoride at the Hygienist

If you refer two family and friends every six months, your cleaning at the Hygienists will be on us!  This is the best of two worlds: You have no out of pocket expense like visiting a preferred provider practice, but more importantly, you will receive a proper detailed clean you will not experience in a shortened preferred provider cleaning.

This offer is for existing patients of Smile Artistry who refer a New Patient to Smile Artistry for a New Patient Exam or Consultation (health fund codes 011 or 014/015).

Conditions of offer:
(1) New Patient must be over 16 years of age
(2) 50% off includes your clean and fluoride treatments only and must be used before the end of June 2012
(3) Offer does not include any further treatment or Dentists examination
(4) The New Patient must  identifiy they were referred by the active patient receiving the 50% off
(5) The 50% off cleaning can be used only after the New Patient has attended for their New Patient Examination or Consultation
(6) Offer is not valid in conjunction with any other offers and is not transferrable
(7) New Patient "free consultations" or "free teeth whitening consultations" are not valid for this offer
(8) The offer cannot be used until credited to the referrers account at the end of the month
(9) This offer is valid when the new patient visits between 1 January 2011 and 30 June 2012.
(10) Offer not transferable for a refund.
