Receive 50% off your cleaning and fluoride at the Hygienist

This offer is for existing patients of Smile Artistry who refer a New Patient to Smile Artistry for a New Patient Exam or Consultation (health fund codes 011 or 014/015).

Conditions of offer:
(1) New Patient must be over 16 years of age
(2) 50% off includes your clean and fluoride treatments only and must be used before the end of December 2010
(3) Offer does not include any further treatment or Dentists examination
(4) The New Patient must  identifiy they were referred by the active patient receiving the 50% off
(5) The 50% off cleaning can be used only after the New Patient has attended for their New Patient Examination or Consultation
(6) Offer is not valid in conjunction with any other offers and is not transferrable
(7) New Patient "free consultations" or "free teeth whitening consultations" are not valid for this offer
(8) The offer cannot be used until applied to the referrers account at the end of the month
